The network of organizations called Long Term Services and Supports offers care for those with a chronic illness or are limited and cannot care for themselves for long periods of time. Services may be medical or non-medical, and can come to an individual’s home, in the community, in an assisted living facility or in a nursing home facility.
Colorado Long Term Care Partnership is an alliance between the private insurance industry and the state government to help Colorado residents plan for future long term needs while remaining as cost-friendly as possible to the patient. Th CLTC website shows that Medicaid does provide assistance with long term care under certain circumstances. The following factors determine an individual’s eligibility for Medicaid services:
- The applicant’s local county department of social or human services
- Evaluation of both income and assets
- Colorado residency, proof of citizenship and provision of a Social Security number
- Meeting the required level of care of long term care services
